To qualify for the incentive, start by completing one biometric screening and selecting two out of the three options listed below: 1. Opt for a biometric screening, which you can schedule at either an on-campus event, a Quest Diagnostics Patient Service Center, or through your primary care provider. The results must be submitted using Quest’s Physician Results Form. 2. Complete the Tobacco Attestation Form, easily accessible in HCM under the “Wellness” tile. 3. Fill out the Primary Care Provider Attestation, also found in HCM within the “Wellness” section. Participating in the Wellness Incentive Program is optional, yet it brings advantages to your health! Keep in mind that any earnings from the program are taxable. This incentive is exclusively available for those who enroll in a university medical plan for 2025.
